This happens after you complete your second run in the Darkwood Dungeon once you defeat Valefar. When you return to your Cult, a follower will come to you and talk to you. They will give you the option to change the name of your cult. Be sure to come up with a name beforehand, as it will be important.
When you get the option to change the cult name in Cult of the Lamb, it becomes your permanent cult name. This is true at least at the time of this writing. Thus, you have one chance to name your Cult. It can be tedious to come up with a name on the spot. However, if you don’t like your cult’s name, you’ll have to start a new save and get to the naming point. That’s why it’s important to be ready with a name. So remember a few names.
